Q:

Dear Dr. We are in Venezuela with the responsibility of taking care of 214 missionaries. A few of them are suffering from back pain. I was wondering if you could send me some information on a few exercises they could do to strengthen their backs.. These young people carry a few books around everyday. I have recommended that they try a fanny pack until of a side pouch or a backpack. I have recommended that crunches will help. Any other information would be a great help. I would purchase your video but things are hard to ship here and we don't always receive them. Thanks for your help. P. W.

A:

Stand with feet shoulder width apart, hands on hips. arch your back (look up at the ceiling) as far as possible untill you cant go back any further. hold for a count of 3 and then stand straight and count to 3.this is one repetition. do the exercise 10 times in a row every waking hour until the pain is better. then reduce the ex. to 4-6 times a day. If you are doing a lot of sitting and bending do the ex. after the activity as well good luck, Dr. L.

Q:

Cronic LOWER BACK PAIN -- Dear Dr Lehrman, I am a 36 year old woman who sustained disc protrusion from a fall at work three years ago. since then I have undergone a disc-ectomy and lastly two and a half years ago I underwent a Anteral / Posterial fusion L5 / S1 a Braddigan cage replaced the disk an two screws were inserted into my back. Since the operation I have had to endure more pain than before the operations. I live in pain 24 hours a day, i have a non existing sleeping routine, I take up to 30 to 40 tablets a day. the major narcotic been physeptone 40 mg a day (methadone), 8 panedine forte a day, anti spasmic medication, something to help me pee, poo, sleep, spasms, pain!!!, anti depressants, vitamins and minerals, just to name a few. my fusion has been unsuccessful. I have tryed lumber facet joint injections with cortisone via my pain management doc. that has stopped due to ill health on the doc's side.i am at my wits end at were to go from here. all I seem to do is pop pills see docs who have no answers for me except get used to it as this is as good as it gets. I'd just like some idea's as to what could be done if anything to help give me some sort of quality of life back. I hope there is an answer out there somewhere. A. P.

A:

YOU HAVE WHAT IS CALLED FAILED BACK SURGERY SYNDROME. IF YOUHAVE A FAILED FUSION THIS MIGHT REQUIRE AN OPERATION TO REFUSE.THE ONLY OTHER CHANCE AT IMPROVEMENT WOULD BE AGRESSIVE EXERCISES WITH THE HELP OF A QUALIFIED PHYSICAL THERAPIST.THERE IS A SPECIAL ECERCISE MACHINE CALLED MEDX WHICH HAS WORKED WONDERS FOR SOME SIMILAR BACK DISORDERS.YOU SHOULD CALL THE MEDX CO. AND FIND OUT WHERE THE NEAREST MACHINE IS TO YOU: 800-876-6339, DR.L.

Q:

Medx Testing -- Dr. Lehrman, I was watching America Health Network the other evening. The show discussed help for chronic back pain. One treatment discussed was the use of Medx equipment to strenghten and test the back muscles. The equipment was developed in Miami. I have suffered from chonic low back pain for seven years. I have tried numerous treatments and discussed treatment possiblities with surgeons and other specialist. The Medx machine may offer new hope for relief from myp ain. I was hoping that the equipment is available some where in Omaha or nearby so that I may have the opportunity to try this new treatment for chronic low back pain. It would be greatly appreciated if you could E-Mail the addresses of the nearest locations of the Medx equipment and those qualified to operate the equipment. Sincerely, L. B.

Q:

Dr Lehrman, My wife has a back problem at this time. She has been going to a chiropractor for some time now. Initially it was obvious her back muscles were out of shape but now the possibility of disc problems seem reasonable. I know people are starting to use exercise to solve a disk problem is it possible your program would be of assistance. She has a sharp pain from the lower back to her ankle on her right side. This is probably a pinching of the psychiatic nerve. The chiropractor is working primarly in her lower back just above the waist region. Thank you, W. A. M.

A:

MY PROGRAM IS AN EXCELLENT GENERAL BACK EXERCISE ROUTINE. IF SHE CONTINUES TO HAVE SYMPTOMS IT WOULD BE PRUDENT TO HAVE A DIAGNOSTIC WORKUP TO R/O ANY SIGNIFICANT DISC PROBLEM, DR. L.

Q:

I visited your Web page and I thought though it is a long shot but I would ask you anyway. Would you happen to know some acupuncturist in North Miami area who would not mind to making house calls for a modest fee? I can also offer an exchange arrangement where I can teach the acupuncturist anything about computers, computer graphics, Web page design and Internet Advertising. Thanks, W. P.

Q:

Hello I am a 29 year male, with chronic pain in my left leg and lower back. I was injuried on my job in sept. 96. I have had all kind of test done. I am in the process of another MRI. I have a mild bulge at L4L5. my left leg goes numb and my anchor hurts all the time. I have had 6 injections in my back but nothing helps. My thigh has a burning feeling at times. I have not been able to work since sept 96 due to the injury. I also at that time of the injury had a double left hernia and a pull left groin. There is no comfortable way to sleep if I stand with my weight on my left leg I feel a pinching in my lower back and my left butt cheek hurts. Do you have any advice? I have not had any therpy yet it hurts to exercise. any advice will be helpful God bless. A. H., NC

A:

IF ALL YOUR TESTS INCLUDING MRI ARE ESSENTIALLY NORMAL THEN SURGERY IS RULED OUT AND YOUR ONLY RECOURSE IS QUALITY PHYSICAL THERAPY. GOOD LUCK! DR. L.

Q:

Arthritis of the back -- After extensive surgery of all 5 lumbar vertabrae, I am still in much pain and recently have been told that I have a herniated disc in my neck, along with osteo-arthritis. Do you have any suggestions as to what steps to take next... more surgery? Exercises? Would appreciate hearing from you! Thanks!

A:

TRY TO FIND A THERAPIST IN YOUR AREA WHO HAS MEDX LUMBAR AND CERVICAL EXERCISE EQUIPMENT, DR. L.

Q:

Back not responding to local treatment -- I am an 85 year old with spine problem causing restricting nerve with concurrent pain in buttocks and legs when walking. If local doctors send you copis of xrays and MRIs may I have your advice for suggested treatment. Have had 2 cortizone shots and am awaiting third. Not effected to correct problem. Thanks for any advice available. I have Medicare and BC/BS. Tired of being a semi-invalid. B.

A:

IT SOUNDS LIKE YOU HAVE SPINAL STENOSIS WHICH IS DIFFICULT TO TREAT. IF YOU ARE HAVING EPIDURAL CORTISONE INJECTIONS AND THEY ARE NOT WORKING THEN THE ONLY ALTERNATIVE IS SURGERY. AT YOUR AGE THAT WOULD BE RISKY BUT CAN BE CONSIDERED. DR. L.
